Please note: submission of the form will alert our conference planning committee of your interest but does not immediately mean you will be selected as a speaker. TXSDPA adheres to the ACCME guidelines for accreditation. As a part of this process all speakers will be required to submit a disclosure statement that ensures transparency between the accrediting body, learners, and planning committee of any relationships a speaker may have with a commercial interest. These relationships include consulting, speaking, advising, employment, and/or ownership of any company that has a financial interest in marketing, selling, or manufacturing of an OTC product, prescription product, diagnostic testing, or equipment involved in patient care. These disclosures are irregardless of the topic for which the speaker is invited and must be disclosed.
